I keep thinking, maybe this is why only Angel was turned into one. If they had turned Spike as well, they wouldn't be allowed to air the episode at all for all the puppet sex. ;-)
Seriously, though, the MPAA is just daft. Even "A Dirty Shame" didn't really need an NC-17.

I never fully understood the whole NC-17 thing. It's ONLY used for sex. I've never heard of any movie getting an NC-17 for violence. As Ron Jeremy (yes, morals from a porn star) pointed out at a lecture last year, it's interesting that it's totally ok to show someone gunning down people, cutting limbs off and blowing shit up, but show two people, even in a committed relationship, having sex, and we can't possibly expose kids or teenagers to it. Oooo...kay.
